Dr Charles Circle presents Pandemic Copay Cooperative, a special and unique program to help
us all, survivors of the Covid-19 pandemic.

It is a simple cooperative, not an insurance contract. Dr Charles Circle joins with its
customers, members, family and friends, to help reduce the burden of medical copay cost.
It is a simple plan of $220/year for $1,000 Copay Reimbursement at $250 every 3 month per
calendar year

There is a 3-month waiting period for each new participant customer.
How to get reimbursement: email clear copies of the copay with date and payment visible,
each 3-month within the calendar year of occurrence
